
All referrals are put through the Referral App
- First time users will need to login (standard District login). Please consider bookmarking the link for easy access.
The app is used by School Based Team (SBT) and case managers for referrals and requests:
- SLP referrals
- School Psychologist assessment referrals, file reviews, completed psychologist designations – LD, MID, MPID
- All other designation requests (IBI, ASD, PDCH, MBS, Deaf or Hard of Hearing, Deafblind, Physically Dependent, Visual Impairment)
- Request for District Collaborative Support for students with Tier 3 needs
After referrals and requests are reviewed, the submitting staff member and administration are contacted via email with the response. This may include:
- designation request outcome
- consideration of a flexible schedule
- mental health planning
- behaviour support planning
- enhanced CPI support
- DBT meeting
- Wraparound team support
- outreach support
